General Summary
Under direction, provides direct service to consumers, family members and designated others; implements program activities; participates in research and program evaluation; provides statistics as requested by administration; link, monitor and coordinate consumer services; coordinate care with physical health care provider(s); familiarity of community resources; develop plan of service using person centered planning process. Performs other duties as assigned.
Essential Functions And Responsibilities
Conducts assessments, service planning, specialized and intensive case management, living skills, training, crisis intervention, advocacy, linking and monitoring for assigned consumers.
Performs home and community-based interventions.
Maintains department productivity and quality measures.
Complies with MCCMH documentation requirements in a timely manner with use of electronic medical record.
Completes required MCCMH trainings and meets continuing education requirements as determined by professional licensing board.
Maintains professional relationships with provider community and consumers.
Maintains cooperative and effective workplace relationships.
Coordinates with family, aftercare programs, and ancillary support services to secure aftercare.
Participates in staff meetings, in-service training and supervision with Clinical Supervisor.
Monitors consumer services to evaluate effectiveness through face-to-face consumer contact, facility visits, consultation with family members/significant others and/or service providers and records review.
Measures the effectiveness of interventions to determine outcomes and identify needs of the consumer.
Qualifications
Required Education and Experience
- Bachelor’s Degree in Social Work, Counseling, Psychology or Doctorate in Psychology or a directly related mental health field from an accredited college or university
- A minimum of one (1) year experience managing services for individuals who are classified as; Severely Emotionally Disturbed children/adolescents
Preferred Education And Experience
- Experience in Early Childhood
- Attended Wraparound training (Michigan Department of Community Health sponsored) 3-day training
Required Licenses Or Certifications
- Meets qualifications as specified in the MCHHS Michigan PIHP/CMHSP Qualifications per Medicaid Services-CMHP (Child Mental Health Professional) qualifications. Must meet one of the three criteria below:
Individual with specialized training or one year of experience in the examination, evaluation, and treatment of minors and their families and who is a psychologist, licensed or limited-licensed master’s social worker, licensed or limited-licensed professional counselor, or registered nurse; or
An individual with a bachelor’s degree in a mental health-related field who is trained and has three years supervised experience in the examination, evaluation, and treatment of minors and their families; or
An individual with a master’s degree in mental health-related field who is trained and has one year of experience in the examination, evaluation, and treatment of minors and their families
- Possession of required state licensure appropriate to position and ability to meet privileging and credentialing guidelines
- Possession of a valid Michigan driver’s license and operable, insured automobile for authorized travel
Preferred Licenses Or Certifications
- Possession of a valid Michigan Chauffer’s license
