This position is included in a bargaining unit of Housing Works’ New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union (“RWDSU”). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a “CBA”) between Housing Works and the RWDSU
Compensation Range: $18/hr - $21/hr commensurate with experience.
Overview
The Housing Works Inc. Reentry Services Program is a collaboration between Housing Works and the Mayor’s Office of Criminal Justice (MOCJ) that provides in-custody discharge planning and post-release, wraparound support services for individuals involved in the criminal legal system in the past three years. The services include: discharge planning, case management, connection to healthcare services, peer support services, vocational counseling, career development, job training and employment placement assistance services.
Reporting to the Community Program Manager, the Peer Navigator will support the service functions for clients actively enrolled in the program. The Peer Navigator will provide individual and group level counseling services, appointment accompaniment, intake and discharge planning support and other office administrative functions. The Peer Navigator will be a self-starter and able to use lived experience working with persons involved in the criminal legal system with substance use, mental health or behavioral health needs.
Primary
Responsibilities:
- Provides navigation and transportation to support to service appointments for clients in need.
- Maintains a caseload of clients newly enrolled in the program for 90 days and provides additional support as needed.
- Facilitates peer support groups and other evidence-based interventions.
- Conducts crisis intervention and conducts 1:1 sessions with clients as needed.
- Conducts community outreach and participates in tabling events to promote program and agency services.
- Assists with conducting intake assessments with newly enrolled clients.
- Maintains accurate and up-to-date case records for outreach and service interventions provided to clients.
- Submits daily reports timely and accurately.
- Assists with greeting and checking in new clients arriving for services.
- Assists program staff with filing, case record updates and other administrative tasks as assigned.
- Participates in clinical case conferences to promote integrated care.
- Perform other tasks as assigned by supervisor.
- Displays Housing Works Core Values consistently.
Secondary
- Provide culturally competent services in accordance to Housing Works policies and practices.
Other functions that may be required with this position include:
