Housing Works fights for funding and legislation to ensure that all people living with HIV/AIDS have access to quality housing, healthcare, HIV prevention, and treatment, among other lifesaving services.

Benefits

We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This position is included in a bargaining unit of Housing Works’ New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union (“RWDSU”). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a “CBA”) between Housing Works and the RWDSU

Compensation Range: $24/hr - $26/hr commensurate with experience.

Overview

Working toward the goals of ending HIV as an epidemic in the United States and improving the long term health outcomes of people living with HIV, Housing Works provides a comprehensive range of medical, behavioral health, vocational and support services to people living with HIV/AIDS as well as innovative prevention support for individuals at high risk for contracting HIV. The Be InTo Health project, a partnership between Housing Works and NYC DOHMH, is a specialized intervention aimed at helping HIV positive Black and Latina cisgender, transgender, non-binary and/or genderqueer women reengage in HIV care and other critical support services. Working as part of the interdisciplinary team that includes a Clinical Social Worker, an RN, a Client Navigator, and the client's external service providers, the Clinical Care Manager will work with clients to assess their needs, connect them to care, collaborate on goals for improved health, and assist with delivering interventions that promote retention in care and activate clients in the management of their own healthcare. Ongoing training on the Be InTo Health intervention will be provided by Housing Works and DOHMH, however, the ideal candidate will have experience working with high risk populations, including but not limited to HIV, homeless, substance use and mental health. Additionally, candidates should have a high degree of cultural competency in working with cisgender, transgender, non-binary and or genderqueer Black and Latino women.

Responsibilities

  • Facilitate client enrollment in the Be InTo Health program, including assisting with intake, outreach/engagement, and eligibility verification.
  • Conduct initial needs assessments and collaborate with clients on establishing achievable care plan goals based on identified needs.
  • Conduct reassessments and update plans of care at regular intervals.
  • Verify engagement in HIV treatment and if needed provide timely reconnection to care.
  • Collaborate with interdisciplinary team on the delivery of individual and group structured health education sessions.
  • Provide direction and support to the Client Navigator in the delivery of enhanced client navigation services, including assistance with access to health care, social services, entitlements, or other services that reduce barriers to engaging in healthcare.
  • Provide and follow through on referral of clients to any needed services.
  • Conference with client’s internal and external providers to coordinate services and assess progress toward care plan goals.
  • Confirm with health care provider that STI screenings are provided semi-annually or as needed.
  • Assist to maintain an environment that is sensitive and welcoming to a diverse client population, including but not limited BIPOC, transgender and non-binary individuals.
  • Participate in departmental Quality Assurance and Quality Improvement (QA/QI) activities.
  • As part of the integrated care team, participate in interdisciplinary team meetings and care coordination activities.
  • Participate in required training or meetings with program administrators and contract funders.
  • Provide crisis intervention as needed.
  • Other responsibilities as needed or required.
