Housing Works fights for funding and legislation to ensure that all people living with HIV/AIDS have access to quality housing, healthcare, HIV prevention, and treatment, among other lifesaving services.

Benefits

We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This position is included in a bargaining unit of Housing Works’ New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union (“RWDSU”). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a “CBA”) between Housing Works and the RWDSU

Compensation Range: $23/hr - $26/hr commensurate with experience

Overview

The Housing Works Inc. Reentry Services Program is a collaboration between Housing Works Inc. and The Mayor’s Office of Criminal Justice (MOCJ) to provide comprehensive, holistic reentry services to individuals with criminal justice involvement reintegrating into the five borough communities. Reporting to the Community Program Manager, the will be responsible for providing in-custody pre-release discharge planning to individuals in New York City Department of Correction custody, community-based case management upon release, and community-based case management services to client walk-ins who meet the program eligibility criteria. These case management services include conducting psychosocial assessments, co-developing treatment plans, facilitate therapeutic soft skills groups, connections to health and wellness services, assistance with benefits and entitlements enrollment and ongoing outreach for the duration of program engagement. The Community Navigator will be solution-focused, experienced in providing clients with wraparound case management services and knowledgeable of NYC resources and benefits available for justice-involved clients.

Primary

Responsibilities:

  • Complete in-custody intake assessments and pre-release discharge plans with clients on assigned housing units.
  • Conduct outreach to connect with participants who did not arrive post-release for services.
  • Refer clients to internal healthcare services and other external referral partners as needed.
  • Maintains essential data for reporting and program evaluation in EiCare and SalesForce including program enrollment, progress notes, treatment plans, referral activities and updating contact information.
  • Promptly complete daily, weekly, monthly and ad hoc reports.
  • Maintain hybrid caseload of clients recently released from DOC custody and community-based walk-ins and provide wraparound case management services.
  • Facilitates community-based therapeutic groups
  • Participates in in-custody and community-based case conference meetings.
  • Works collaboratively with team members, agency colleagues and DOC personnel.
  • All other tasks, or projects, as assigned by supervisor.
  • Display Housing Works core values in all functions of the position.

Secondary

  • Provide culturally competent services in accordance to Housing Works policies and practices
  • Promotes Housing Works core values consistently.
  • Ability to work with clients with diverse clients with varying levels of service needs.
  • Works collaboratively with other team members within the Justice Initiatives Department and cross-agency.

  • Attend and participate staff meetings and trainings as scheduled.
  • Responsiveness to emails, texts and phone calls.
  • Ability to adapt and successfully uphold job functions in a full remote workforce or hybrid in-person remote workforce environment.
