I. Major Responsibilities:

  • Implements program policies and procedures to maintain daily structure and consistency within the program and maintain compliance with all licensing standards.
  • Assesses clients basic physical, social, educational, legal and service needs and coordinates care with other inter-agency and community-based providers.
  • Works with multi-disciplinary treatment team to develop and update integrated treatment plans and recommendations including all service needs.
  • Develops appropriate comprehensive and appropriate aftercare plans while working with clients and members of multi-service team providers.
  • Conducts outreach and creates pathways for referrals. works closely with other providers to receive referrals; may conduct admissions assessments.
  • Assists clients in engaging next level of services.
  • Coordinates and provides transportation as needed.
  • Participates in multidisciplinary team to discuss client progress.
  • Makes referrals and assists clients with transition to behavioral treatment providers.
  • Leads skill development, educational or recreational group activities as assigned.
  • Attends program staff and clinical team meetings, supervision, trainings and all other required program meetings.
  • Documents treatment provided in EHR (Electronic Health Record), assessments, follow up interviews, case conferences, behavioral issues on the unit, coordination with other agencies or programs, referrals, clinical team treatment decisions, discharges and others.

II. Position Qualifications:

License/Certification/Education:

Required:

  • Bachelor’s degree.
  • Satisfactory CORI background check.

Preferred:

Grade C02: Case Manager-II Bachelor’s degree in psychology or human services or a closely related field is preferred.

Experience/Skills:

Required:

  • Ability to develop a working knowledge of CHL’s service lines to triage individuals being assessed for services.
  • Excellent oral communication and computer skills.
  • Ability to multitask. Must possess time management skills with a keen sense of prioritization and follow through.
  • Requires personal and professional accountability, self-management, and initiative. Professional integrity and cultural competency.
  • Ability to use office equipment, including copy machines, computers, printers, telephones.
  • Ability to organize complex tasks and meet deadlines.

Preferred:

  • Prior experience in a behavioral health or social services setting.
  • Basic knowledge of CHL services and insurance requirements and reimbursement for these services.
  • Working knowledge of Electronic Health Records (EHR).

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Position Summary - Supportive Employment And Education (SEE) Specialist:

As a member of CHL's Screening and Treatment of Early Psychosis (STEP) Team, you would be working in one of a handful of clinics in Massachusetts serving clients experiencing First Episode Psychosis (FEP) and their families. We provide a treatment modality called Coordinated Specialty Care (CSC) - Specifically NAVIGATE; which includes Individual Resiliency Training (IRT), medication management, Supportive Employment and Education (SEE) Services, Family Education (FE), Peer Support, and Recovery Coaching. We are the only CSC team affiliated with MAPNET (Massachusetts Psychosis Network for Early Treatment) located in Worcester County. You would be a member of a collaborative and integrated team, and be able to receive unique educational opportunities via trainings and learning collaboratives with experts in the field of FEP throughout the state and country.

As a Supportive Employment and Education (SEE) Specialist - you would assist young adults in achieving their employment and education goals. These may include a return to school or admission/readmission college, as well as seeking employment for the first time and/or returning to work. You would be collaborating with individuals and their families to assess employment and educational needs, and working alongside other CSC team members.

It is expected that you attend the weekly STEP Team meeting, and a monthly SEE Supervision call with individuals from the NAVIGATE Team.

Must have a valid Driver's License, own a vehicle, and be able to transport clients.
