Occupational Therapist Case Manager - ABI Outreach Team

1 FTE (35 hours/week) – Permanent

About the Position:

We require one full-time Occupational Therapist (OT) Case Manager to work as team members of the ABI Outreach Team. The ABI Outreach Team is an interdisciplinary team of professionals that supports people with an Acquired Brain Injury, as well as their families, in their homes and other community settings as needed. The ABI Outreach Team is AN established within a partnership between SCHC and Cota. The team is composed of professionals who work cohesively to support the complex healthcare needs of this client population, their families and caregivers.


Reporting to: Manager, Community Health and Support Services Manager


Key Areas of Responsibilities:

  • Conducts visits in the clients’ homes, hostels, hospitals and other community settings.
  • Ability to do assessments and create care and treatment plans based on client and family goals.
  • Thorough familiarity with therapeutic methods and practices based upon a health promotion/disease prevention model;
  • Ability to work collaboratively with the interdisciplinary health care team and independently when necessary;
  • Willingness to develop client education materials;


Community of Practice Partner Engagement:

  • Relationship building across inter-disciplinary teams and community partners.
  • Advocating for clients’ needs with other health care professionals.
  • Provide consultation to internal staff members as well as community professionals from various organizations with the goal of enhancing skills and meeting client goals.
  • Operations
  • Utilize effective communications strategies; shared care plans; and EHR charting
  • Meet performance indicators and participate in data collection.


Qualifications (Knowledge, Experience):

  • Current (or eligible for) registration with the College of Occupational Therapists of Ontario.
  • Competencies (Skills and Attributes):
  • Oral and/or written fluency in other languages would be considered an asset
  • Proficiency in the use of computers and various software applications, including EHR applications.
  • Knowledge of the health care system as it relates to ABI preferred.
  • Demonstrated problem solving and critical thinking.
  • Proven time management and ability to work autonomously.
  • Excellence in communication, partnership building and inter professional care commitment.
  • Demonstrated commitment to continuous learning and professional development.


Working Conditions:

  • Travel throughout Scarborough, access to a vehicle an asset.

  • Hybrid work.
  • Standard working hours


Remuneration: $33.04 – $39.80

Band: D
