Job Description
Coordinates the total Plan of Care for the patient in the Home Care setting assessing, planning, implementing and evaluating. Provides leadership for the Health Care team by working cooperatively with other team members in maintaining standards of professional nursing practice in the Home Care setting.
Minimum Or Preferred Qualifications
Associates Degree, plus 1-5 years of relevant experience, Current license required.
Salary Range
$28.57-$31.07 per hour *Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.*
