TITLE: Case Manager
QUALIFICATIONS: Current PEL (Public Educator License) with endorsement in School Psychology or an LBS1.
JOB SUMMARY: Facilitate and coordinate special education meetings during the case study evaluation process and IEP meetings
REPORTS TO: Principal(s), Assistant Superintendent of Special Services, and other district administrators.
TERMS OF EMPLOYMENT: Salary based on Chicago Heights School District 170 and American Federation of Teachers, AFT Local 604, Chicago Heights Council contractual agreement.

Responsibilities
- Will consult with staff members and principals on potential referrals, attend MTSS meetings as needed, staffing schedules, and follow-up recommendations.
- Will receive, review, and process all special education referrals .
- Will schedule, conduct and participate in the following:
- Collaboration consultation conferences
- Initial case studies
- Re-evaluation conferences
- Eligibility Conference
- IEP (Individualized Education Plan)
- Will coordinate with staff the process of collecting data needed for case study staffings, screenings, and psychological consultations.
- Will inform all staff members involved, including the principal and Director of Special Education of all case study conferences at least one week in advance.
- Will complete all staffing papers and place appropriate copies in Manager’s and Teacher’s Special Education files.
- Will review incoming special education student folders and keep an up-to-date account of all special education folders.
- Will coordinate all out-of-district staffings (Packet B) with non-public schools.
- Reevaluations - Will establish a list of all special education students due for reevaluation that school year and distribute appropriate forms for data collection for completion.
- Will forward all necessary information to parents, other outside agencies when necessary who are unable to attend case study conferences.
- Will forward to the Special Education Director the district’s copy of all case study staffing reports: Staffing Notices, Eligibility, Psychological, Case Study Packets, etc.
- Will participate in and assess problems with student’s referral for preschool screenings through the use of testing and other diagnostic techniques.
- Will be a district liaison to the Director of Special Education.
- Will coordinate and schedule Annual Reviews for assigned schools.
- The Special education caseworker will provide input to the Director of Special Education that is beneficial for district programming.
- Will provide other case manager responsibilities as assigned by the Director.
