Under general supervision, this position is a dynamic component of Community Living Services Intake and their related processes. This position is responsible to serve a lead role within assigned tasks, provide recommendations to management as well as provide direction to others performing work within related processes. Case specialist positions in the Department supervise, administer, and implement client based programs, involving direct contact with people served by the Community Living Services program area, and contain multiple types of responsibilities within each role.

The primary function of this position is to speak with individuals via phone and complete preliminary screening tasks in order to provide information, resources, and referrals, as well as provide support to Social Workers within Community Living Services. This position will require significant interaction with community contacts, individuals seeking services, Social Workers, Case Managers, and others to ensure callers receive sufficient information to determine their next steps. The Case Specialist in Community Living Services will support the day to day functioning of intake and will require strong organizational and multi-tasking capabilities.

Minimum Qualifications

NEED TO HAVE QUALIFICATIONS:

  • Bachelor's degree in human services or a related field

OR

  • Combination of related education and experience with social services to total four years

Required License(s)

  • Valid Driver's License

Duties & Responsibilities

  • Assist with the answering of phones and returning/triaging calls in a timely manner.
  • Determine client need, level of service, and eligibility for identified client; provide education and support to client about program services and how to maintain benefits.
  • Locate financial/insurance information using MAXIS to support caller inquiries.
  • Review online information submission forms and make return calls to those seeking assistance.
  • Recommend policies and procedures to management, assist in the development of new programs, may establish or revise existing program guidelines.
  • Handle confidential client records and other information as needed, and collect and track initial client demographic data
  • Research and compile information from a variety of sources and prepares statistical analysis and reports on various programs
  • Demonstrates a commitment to support cultural diversity County wide, actively supports the County's inclusion, diversity and equity initiatives, and works to address the needs of diverse and underserved communities
  • Develop databases or spreadsheets to track program/intake information and develop queries for reporting purposes
  • Other duties as assigned

WORK ENVIRONMENT: Work is performed remotely, in a County office building, and various sites within Dakota County. Lifting requirement of up to 20 pounds on an occasional basis. Types of equipment used include, but are not limited to, automobile, computer, telephone, copy and fax machine.
