Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a âCBAâ) between Housing Works and the RWDSU
This position requires applicants to successfully complete a background check through the New York State Office of Children and Family Services when it supports the mandate under Section 424-a of the Social Services Law for legally authorized agencies to inquire of the Statewide Central Register of Child Abuse and Maltreatment (SCR) as to the existence of any indicated reports of child abuse and maltreatment against an applicant prior to employment, certification, or licensure in the childcare field. All background checks will be conducted and reviewed in accordance with applicable federal, state and local law.
Compensation Range: $65,000 - $70,000 yearly commensurate with experience.
Overview
The LMSW therapist provides specialized Substance Use and Mental Health Treatment Services for a combined Integrated Outpatient Services (IOS License) and Outpatient Substance Use (A32 OASAS 822) panel of care recipients at the East New York Health Center. The LMSW therapist establishes therapeutic relationships with IOS and OASAS enrollees and provides clinical care through the delivery of individual therapy, groups and integrated collaborations and advocacy to promote optimum wellness with a focus on trauma-informed care, harm reduction and prevention. The LMSW therapist will provide care to people with diagnoses of mental health and/or substance use from multiple underserved populations, including persons who have ongoing mood and/or perceptual disorders;LGBTQ individuals; the unhoused or unstably housed; persons with chronic health concerns; and persons involved in the criminal justice system. The LMSW therapist reports to the Assistant Director and is responsible for providing multiple aspects of clinical services to clients through varied clinical assessments ofhigh-riskbehaviors, mental health/substance use treatment needs, crisis intervention, individual counseling and group facilitation. The LMSW is responsible for the overall maintenance of a therapeutic environment in the Clinic.
Responsibilities
- Maintain confidentiality of records relating to clientâs treatment
- Completes initial andepisodic psychosocial,mental health and substance use assessment, treatment plans, and timely discharges.
- Assess each client for risk level and complete safety planning/monitoring as needed
- Counsel clients, individually and in group sessions, to assist in addressing presenting goals and needs, adjusting to life and making positive changes.
- Complete comprehensive assessments to support appropriate DSM diagnosis
- Act as client advocates to coordinate required services such as detoxification, rehabilitation, respite programs, etc
- Support with emergency problems in crisis situations using de-escalation techniques and clinical engagement with appropriate follow-up and consultation with supervisor and interdisciplinary team.
- Complete all documentation and billing within 24-48 hours
- Engage in outreach to clients who do not attend scheduled appointments and document appropriately
- Collaborate with other staff members in interdisciplinary team to perform clinical assessments and develop and update treatment plans.
- Evaluate clientâs physical or mental condition based on review of client information, use of prescribed medications and attendance to provider appointments
- Assess the effectiveness of therapy programs and clientâs progress in resolving identified problems and moving towards defined objectives
- Modify treatment activities and approaches as needed to comply with changes in client's status
- Remains aware ofall programpolicies thatensure quality comprehensive care and to meet the regulatory agenciesâ guidelines.
- Provides individual counseling and facilitation of varied clinical and psycho-educational weekly groups.
- Perform toxicology assessments as needed.
- Completes weekly, monthly program reports, including familiarity with STARS, HRA-HASA Harm Reduction Report, parole/probation/ACS and collaboration with the courts around client mandates.
- Performs other duties as assigned by Assistant Director of IOS/OASAS services.

- Participate in monthly continuing education
- The Therapist will have access to PHI during the course of his/her work activities. Incumbent will use this information to provide psychiatric therapy as needed in compliance with all regulations and guidelines relative to established plan of care. ALL HIPAA, corporate compliance, and program guidelines must be maintained and kept at all times.
- Attendance of in-service training required.
- Participatesin agency wide continuous quality improvement activities aimed at improving health outcomes for program participants
- Attends agency advocacy efforts to assist the continued efforts to end the dual crises of homelessness and AIDS and other agency focuses.
