Housing Works fights for funding and legislation to ensure that all people living with HIV/AIDS have access to quality housing, healthcare, HIV prevention, and treatment, among other lifesaving services.

Benefits

This position is included in a bargaining unit of Housing Works’ New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union (“RWDSU”). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a “CBA”) between Housing Works and the RWDSU

This position requires applicants to successfully complete a background check through the New York State Office of Children and Family Services when it supports the mandate under Section 424-a of the Social Services Law for legally authorized agencies to inquire of the Statewide Central Register of Child Abuse and Maltreatment (SCR) as to the existence of any indicated reports of child abuse and maltreatment against an applicant prior to employment, certification, or licensure in the childcare field. All background checks will be conducted and reviewed in accordance with applicable federal, state and local law.

Compensation Range: $68,000 - $74,000 yearly commensurate with experience

Overview

The Social Worker provides comprehensive mental health services to clients who are both HIV-positive and have a mental health condition, many of whom also experience co-occurring substance use issues, homelessness, complex health issues, and/or identify as LGBTQQ.

Responsibilities

  • Maintain confidentiality of records relating to client’s treatment.
  • Determine risk factors for mental health and substance use concerns of clients and significant others by conducting short term counseling sessions and/or support groups providing on-going assessments for mental health and substance use and coordinating delivery of services between service providers (i.e. primary care providers, psychiatrists, day programs, rehab/detox programs, hospitals, and mobile crisis units) and clients.
  • Ensures that all services are maintained at program capacity by coordinating all phases of the intake process and assessing and evaluating the effectiveness of counseling services for individuals enrolled in the program.
  • Maintains program in contractual compliance by documenting movement of treatment for the client in progress notes and completing reports for funders utilizing existing program forms to gather client data.
  • Assists in stabilizing clients in crisis situations by assessing emergency situations, developing a plan of action that may include intervention by other staff members, making recommendations to program staff and administrators as to how to proceed and collaborating and referring to mobile crisis units, emergency housing or other health and service providers.
  • Ensures that the pertinent needs that impact clients are presented to staff members by participating in team meetings and case conferences.
  • Prepare and maintain all required treatment records and reports.
  • Counsel clients, individually and in group sessions, to assist in overcoming dependencies, adjusting to life and making changes.
  • Develop and implement treatment plans based on clinical experience and knowledge
  • Collaborate with other staff members to perform clinical assessments and develop treatment plans.
  • Evaluate the effectiveness of counseling services and client’s progress in resolving identified problems and moving towards defined objectives.
  • Modify treatment activities and approaches as needed to comply with changes in clients status.
  • Promote awareness and understanding at the site level of available services within the department.
  • Comply in the development and execution of Quality Improvement Initiatives in collaboration with agency’s QI Team.
  • Participate and support advocacy efforts to end the twin crises of homelessness and AIDS.
