The Physician Resource Nurse oversees patients’ care and evaluates the appropriateness and outcome of care in conjunction with the patient care team. As a Physician Resource Nurse you support and act as a resource to the physician/APN and health care team. The Physician Resource Nurse strives to achieve positive patient care outcomes throughout the disease process by practicing according to the standards of care and procedures and policies provided by the University of Chicago Medical Center, and the State of Illinois.
Essential Job Functions
- Plans, monitors and coordinates patient care using the plan of care or clinical pathway.*
- Coordinates care between inpatient and outpatient settings
- Collaborates with the patient, family, care team and multi-disciplinary team to assure appropriate, safe, cost-effective, timely and efficient care.*
- Identifies patient and family needs for consultation with other multidisciplinary specialists and assures needs are met.*
- Assures patient and family educational needs are met.*
- Demonstrates knowledge of cardiovascular/advanced heart failure patient care
- Able to work independently
- Collaborates with team (providers, APPs, and nurses) on a daily basis
- Rounds on patients in hospital for education and assessment of patient needs
- Collaborates with specialty pharmacies for care and treatment of prostacyclin's
- Takes on call for questions and drug titration
- Educates hospital nurses in care of pulmonary hypertension patients
- Attends clinic with physicians
- Manages in basket for pulmonary hypertension patients and advanced heart failure patients
Required Qualifications
- Current licensure as a Registered Nurse from the State of Illinois.
- Current Basic Cardiac Life Support Resuscitation (BCLS) certification
- Two years experience in advanced heart failure management
