Position Objective
To engage with clients in the community, medical, employment, education and home setting to increase the focus of individual's successful participation with program areas such as social services, corrections, community health, medical, public assistance, employment and housing. This position may involve handling a large volume of phone calls and documents to help clients connect to various programs, services, and community resources. It also responsible for research, assisting clients, and navigating services through a person-centered inclusive approach and will use effective and innovative outreach and engagement strategies to achieve self-sufficiency for families and individuals.
Essential Duties And Responsibilities

- Helps clients figure out which programs and services can help them become self-sufficient and works with them to gather and verify necessary documents.
- Reaches out to clients to make sure they keep their program services active.
- Screens phone calls and incoming documents to initiate a case for clients to access assessments for program services.
- Sets an example for how to communicate with state agencies, banks, landlords, employers, childcare providers, other agencies, and staff.
- Interviews clients, their families, and community resources to coordinate and set up services, and to verify information needed for program and service eligibility.
- Keeps records on clients' eligibility for programs and services and ensures documentation of activities in a variety of systems.
- Works closely with clients and a team of professionals to help clients achieve positive outcomes in social services, probation plans, and other objectives.
- Helps arrange and schedule meetings for clients; may need to transport or accompany clients and their families to service locations.
- Attends work regularly and in a timely manner.
- Performs other duties as assigned or apparent.
Minimum Qualifications
Minimum Qualifications and Desirable Knowledge, Skills and Abilities
- High School graduate or equivalent
- One year of post-secondary formal education and two years of progressively related experience providing service delivery to community members related to interpreter services, public assistance, community health, education, or human services OR four years of progressively related experience providing service delivery to community members related to interpreter services, public assistance, community health, education, or human services
- Valid Minnesota Driver's License or ability to obtain within 30 days of employment
- Reliable means of transportation and be able to travel to multiple locations around Stearns County

Work Environment and Physical Demands
The primary responsibilities for this position will be performed approximately 100% of the time indoors. Will require frequent travel to off-site locations and minimal attendance at evening meetings. Will have infrequent exposure to health hazards and accidents; requires a low level of physical effort.

Work is performed under the general supervision of the Human Services Supervisor.

Hours of Work
This position has a normal work schedule of 8:00 a.m. to 4:30 p.m., Monday through Friday; may include some hours outside of normal work hours on occasion. Potential for a hybrid work environment and flexible work hours.
