Job Description
This position is responsible in assisting the timely coordination of admissions from acute care settings to the Kaleida Health Acute Medical Rehab Unit(s) and/or long term care and sub acute care settings
Education And Credentials
HS - High School Diploma or GED required. BS - Bachelor of Science nursing preferred. Registered Nurse required upon hire. FIM Function Indp Meas required within 90 days. FIM Certification (Functional Independence Measure) - obtained through successfully passing the Licensed Personnel Exam through the on line credentialing system on Uniform Data System for Medical Rehab (UDSMR.org), within 90 days of hire or appointment to the position, for medical acute Rehab settings only. REQUIRED - Certification must be renewed every 2 Years. Patient Review Instrument (PRI)required within 90 Days.
Experience
1 year of experience in medical acute rehab within the past 5 years for medical acute rehab settings only preferred. (This requirement only applies to Acute Care setting preferred).
